The Gaps Are Familiar

You know where failure begins:

• A student, volunteer, staff member, parent, or congregant reports concerning behavior, but no one owns the assessment.

• A threat, custody conflict, domestic-violence concern, or online post is treated as an isolated problem.

• Doors, cameras, visitor procedures, and emergency communications have never been tested together.

• Staff and volunteers have received training, but no one knows whether they can act under stress.

• A reunification, shelter-in-place, evacuation, medical, or crisis-communications plan has not been exercised with local responders.

• Leadership assumes someone else is tracking the concern, the gap, or the corrective action.

A policy does not stop harm. A safety plan does not protect people if it is not current, understood, practiced, and accountable.

The Questions Will Come

After a violent incident, missing child, medical emergency, severe-weather event, fire, cyber disruption, or public crisis, the questions will be direct:

• What warning signs were received?

• Who reviewed them?

• What risks had already been identified?

• What safety gaps were known?

• Who was assigned to fix them?

• Were staff and volunteers trained?

• Were procedures tested with the people who had to use them?

• Can leadership prove the work was done?

If the answer is unclear, the organization was not ready.
